Your First Visit


Initial Visit Information

Your initial appointment will consist of a complete examination and consultation which will include:

  • Any necessary x-rays, photos and models
  • A thorough examination of your teeth and gums
  • A review of your previous dental and medical history
  • Discussion about your concerns
  • A consultation with all treatment options thoroughly explained

* If there is a complex medical history or multiple dental problems, a more extensive evaluation, work up and second consultation appointment may be necessary.

Please assist us by providing the following information at the time of your initial appointment:

  • Any dental records and x-rays that you may have. If your records and x-rays are at another office, we would be happy to call them and have them sent to us.
  • A list of medications you are presently taking.
  • Some form of photo ID
  • If you have dental insurance, bring your ID card. Please also bring your medical ID card if you have medical insurance. The recent changes in healthcare sometimes now allow certain dental procedures to be processed through medical carriers and having this information available could simplify your claim processing in the future.

All patients under 18 years of age should be accompanied by a parent or guardian.


Dental Insurance

You can expect most insurance companies to reimburse you for a portion of our fee. That portion is determined by the contract between you or your employer and the insurance company.

Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a courtesy to you, we will help you process your insurance claims in order for you to receive this maximum benefit. We will also provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.

At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you, a contract we feel morally obliged to honor.


Financial Options

Our treatment plans are highly individualized as well as our financial arrangements with our patients. For your convenience, we accept checks, cash, and major credit cards. We offer a variety of payment options including participation with CareCredit, which provides some patients a flexible option to cover health care costs. Click here for more information about CareCredit. Whatever method you will be using, our treatment coordinator will work out a plan suited to your individual needs.
